I have been using Unique Art Services with great success. They charge me a flat rate that is pretty reasonable. I don't work for them, just have used their services.

having 'flat rate' be your criteria for a digitizer isn't a great start. find a good one...it can make or break your business. i understand that it makes the ease of quoting designs to your customers a bit easier, but that shouldn't be the 'end all'. be prepared for a lot of "try us!" unfortunately, the title is an invitation for a bunch of spam.
