Default error code 291 tajima 6 head

My moms machine has been giving her this error code 291 . after doing a little research i am seeing its a code for upper thread break but we have checked all of that and nothing seems to be wrong with the threading it is happening on heads 4 and 6 i would press start and it will begin and then 2 to 5 seconds later error but if i hold the green start button for slow mode i can probably literally finish the full logo with no stoppage. any help would be great also please let me know if you need any info from me thank you again in advance.

Hi, I think you should check following things.

Tension Spring position ( sometime its moved up and touching upper copper plate.
Head PCB ( which is installed on top of the head. you can replace with other heads which is working good)
Head PCB Cable which is connected with main cable on back side of machine. some time its got bad or damage.

Which model do you have? There are settings in the control panel as well as a physical setting that need to be checked first.
