Default Getting a loop when machine starts its first stitch

I am doing some shirts and every so often I am getting an 1/8'' loop when the machine does it's first couple of start stitches. Doesn't do it all the time, is in a different location, tension test is good, hook timing and depth look good. What am I missing?

We did another FOX tension test and noticed it was erratic so we ended up changing the bobbin case and that seems to fix it.
