Default Problem with embroidery after washing

Why is it that I notice some items gather up a little after they have been washed?

maybe not hooped tight enough..
wrong backing
stitch heavy?

this is EXACTLY why we tell our clients NOT to wash embroidered garments.. think of them as disposable and come back for as many reorders as they need! No, seriously I do think the backing and thread tensions are part of the problem, but the major issue is probably the digitizing. Too many stitches.
