CPSIA in a nutshell is an absolute mess .... there are a million grey area's and things are just in general a pain. I refuse decoration on anything "youth" unless its DTG.
Generally, you will have to use tear away stabilizers and certain thread. I do believe you have to use the fire retardant thread, but don't hold me to that.
I'm not sure about complete contact info, but, yes, you do need at least something that clearly identifies you as the "manufacturer"
